Job Summary

The Heart Failure Nurse Navigator is a specialized nursing role focused on providing comprehensive care and support to patients with heart failure. The nurse navigator serves as a key point of contact and resource for patients, their families, and the healthcare team, helping to coordinate care, educate patients about their condition, and promote self-management strategies. This position requires a deep understanding of heart failure management, excellent communication skills, and the ability to collaborate with multidisciplinary teams.

Job Requirements

* Education* Required: Graduate of an approved nursing program* Preferred: Bachelor's or Master's degree in nursing, business or health related field

* Experience* Required: Three years clinical experience in cardiology or heart failure management, preferably in an acute care or specialized outpatient setting

* Skills* Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.* Proficiency in using electronic health records and other healthcare software systems.* Ability to work independently, prioritize tasks, and manage a caseload of patients effectively.* Demonstrates commitment to professional development, staying current with research and advancements in heart failure care.

* Licensure/Certification/Registration

Required: MO RN License* Preferred: Specialized certification in cardiac nursing, heart failure management, or a related field (e.g., Certified Heart Failure Nurse)
