Functions independently to perform age-appropriate history and physicals for Pace population Orders, interprets diagnostic tests relative to patient's age-specific need and co-morbidities Implements interventions to support patient stability Actively participates in Interdisciplinary Team regarding Care Planning Assists with training of APRN Students, PA Students, Nursing Students, Fellows, and Residents Acts as Proxy to MD on Ethics Committee, Quality Assurance Committee, and other Committees as needed. Completes Assessments as per PACE Regulations Documents services in the Electronic Medical Record in a timely manner Serves rotation of On-Call status Provides consultation and direction to PACE Nursing staff as needed.
SUMMARY OF RESPONSIBILITY - Job Summary• Responsible for providing direct patient care, diagnosing and treating health problems and coordinating health care for PACE population in accordance with State and Federal rules and regulations. Clinical Management is conducted in collaboration with other team members. Serves as Consultant and resource to On-Call Professionals. Models appropriate behavior as exemplified in the Complete Health with PACE mission and values.
MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience• Graduate of an Accredited School of Nursing. Graduate of an Accredited Nurse Practitioner Program with a current APRN Licensure. Current in BLS and/or ACLS. Computer Skills. Eligible for Hospital Privileges. Minimum of one year's experience providing services to Frail Elderly population. Clear of Communicable Diseases. Current on Immunizations. Valid Arkansas Driver's License.
ESSENTIAL FUNCTIONS
1. Punctual and regular attendance.
2. Job duties must be performed on a Baptist Health campus, nursing home(s) and PACE participant's home.
3. Participates in the overall development of Plans of Care for PACE population.
4. Functions independently to perform age-appropriate history and physicals for PACE population
5. Adequately and timely documents in Electronic Medical records as required by program.
6. Skin and Wound Care
7. Infection Control Techniques
8. Serve On-Call, alternating with other APRN and MDs
9. Orders, interprets diagnostic and therapeutic tests relative to patient's age-specific needs and co-morbidities.
10. Prescribes appropriate treatment modalities, performs medication reconciliation on PACE population.
11. Assists in coordinating services provided by specialists and contracted services to ensure smooth transitions of care as needed.
12. Monitors effectiveness of interventions and adjusts Plan of Care accordingly.
13. Meets with supervising MD on a regular basis
14. Complete All assessments as required by policy.
15. Functions as an effective Interdisciplinary Team member as needed.
16. Facilitates staff, patient, and family decision making by providing education during hospitalization and instruction for post discharge.
17. Ensures Medical Services are provided in compliance with PACE Federal Regulations as they pertain to Medical and Clinical components of care
18. Serves on Ethics, Quality Improvement Committee, and other Committees as Medical Director's Proxy as needed.
19. Serves as a Liaison with Specialists in the Community.
20. Provides Consultation and Direction to PACE Nursing staff as needed
