Reporting to Regional Manager of Clinical Operations

Employment Type: Full-Time. This position is also hybrid / remote.

Invision Sally Jobe (ISJ) is a network of imaging centers built and managed through a partnership between Radiology Imaging Associates and HealthOne. Our imaging centers are conveniently located throughout the South Denver area and offer a variety of exams using state of the art equipment. Our services include MRI, CT, digital mammography, ultrasound, pain management, DXA, X-Ray, and image guided biopsies. Our mission is to improve the health of patients in the communities we serve by providing them with the highest quality imaging and associated medical care.

Summary of Position:

The nurse navigator interfaces with the breast health team and assists to provide comprehensive care to patients who are diagnosed with breast cancer and/or high risk breast conditions requiring treatment/follow-up. The nurse navigator works collaboratively with the radiologists, genetic counselors, referring physicians, treating physicians, and treatment navigators to ensure that the patient gets seamless breast care for optimum outcomes. The nurse navigator assumes the responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team and the patient to identify and meet the physical and emotional needs of the patient. The nurse navigator demonstrates clinical competency and promotes the optimal health, well-being and safety through use of nursing processes in accordance with patient care standards. This position also provides clinical nursing functions as required and participates in maintaining a safe, cost-effective patient care environment.

Job Responsibilities:Provides coordination of care for the newly diagnosed patient, including those with breast cancer, high-risk and benign findings.Collaborates with the breast radiologist to inform the patient's referring physician about the diagnosis and confirm the referring physician's preference regarding orders for additional imaging/procedures and referrals to specialists.Calls all patients with a diagnosis breast cancer or a high risk lesion to inform them of their biopsy results. Provides education about the immediate diagnosis and the plan for subsequent diagnostic evaluation, which may include additional imaging (i.e. breast MRI, ultrasound correlations, and/or other exams), surgical/medical oncology consultations and genetic counseling.Follows up with newly diagnosed patients after diagnosis to discuss the nurse navigation role, confirm/schedule additional appointments, and act as resource to answer questions and address concerns.Provides continual support to newly diagnosed patients through subsequent diagnostic evaluations and any recommendations for further biopsies. Assists with communication among the multi-disciplinary team during this process.Calls patients with benign biopsy results.Assist in nursing duties as needed to include checking and monitoring vital signs, blood draws, wound care and other nursing responsibilities as needed.Responds to emergency care as needed within the radiology department.Closely collaborates with the Sarah Cannon nurse navigator team and provides a hand off of the patient for a smooth transition.Performs follow up calls to patients post biopsy with positive and benign results.Assists the patient and their family in overcoming barriers by providing information and referral to appropriate internal and external resources.Builds longitudinal relationships with referring physicians and documents their preferences related to orders for additional imaging/procedures and referrals to specialists.Educates referring physician offices about the HealthOne-accredited multi-disciplinary programs that are available.Documents all patient contacts, treatment providers, appointment dates, surgical procedures and treatment updates utilizing the mammography reporting system (MRS) system.Attends relevant case conferences and administrative meetings on a regular basis.Actively participates in community outreach programs and activities.Works collaboratively with the team to develop, evaluate, and improve relevant processes.All job duties and procedures performed must be within the scope of practice for this position based on the terms of the professional license.Must be able to provide documentation and demonstrate training and competence to provide specific services when required.Maintain values congruent with the Code of Ethics as well as adhering to national, institutional and /or departmental standards, policies and procedures regarding the standards of care for patients.Acts as a professional role model for all staff demonstrating ethical, legal, professional, and empathetic behavior.Performs other related duties as assigned.

Compensation for this role is between $75,000.00 and $102,200.00 per year based on experience.

In accordance with Colorado law, the range provided is Invision Sally Jobe's reasonable estimate of the base compensation for this role. The actual amount may be higher or lower, based on non-discriminatory factors such as experience, knowledge, skills, and abilities. This position will receive applications on an ongoing basis and will remain open until filled.

Experience/Skill Requirements:Advanced knowledge of breast cancer screening and diagnostic toolsProven success in protocol/pathway-driven environmentAble to navigate patients through various multidisciplinary breast care programsAble to assess, plan, intervene, and evaluate the physical, emotional, and educational needs of the patientsStrong written/verbal communication and interpersonal skills to effectively interact with patients, co-workers, referring physician groups, and radiologistsExcellent problem solving and organizational skillsAble to work efficiently and effectively both independently and as a member of a teamStrong computer and systems skills.Able to manage multiple duties simultaneously in a fast-paced environment

Education Requirements:Graduate of an accredited school of Nursing; valid registration as a Registered Nurse in the State of ColoradoCBPN-IC NCBC, AAMSN, and ACLS certifications preferred
