Are you a new grad or experienced post-acute care nurse practitioner (NP) or physician assistant (PA) who aims to make a difference? TeamHealth needs someone like you! We are seeking an NP or PA to work in a skilled nursing facility rounding 2-3 days/week. This opportunity includes the flexibility to set your own schedule. We have no start/stop times so you can round in the morning or in the afternoon and still have great work-life balance.
Key Responsibilities:
* Perform primary care in a skilled nursing facility setting for short term and long-term patients* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care* Order and interpret lab results* Prescribe and monitor medications* Consult with physicians and coordinate patient care with other healthcare providers* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery* Promote healthy lifestyle changes

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $45,000 to $65,000 annually with no cap on productivity income potential. This is a part-time 2-3 day/week opportunity
