Job Description

This role will work in person in our Clifton, NJ office. This position will work with 50% Breast patients as well as GI, Lung and Prostate patients.

Breast Navigation: (In Person Position)

* Navigates breast patients across the continuum of care beginning with the diagnosis of Breast cancer through cancer survivorship, coordinating care with staff in Radiology, Surgery, Plastics, Oncology, Gynecology, Genetic Counseling, Internal medicine and Behavioral Health, and refers to community resources to facilitate timely diagnosis, treatment and survivorship.* Adheres to key Touchpoints for Breast Navigation (attached) including Introduction to Nurse Navigator at time of cancer discussion, Post-operative care planning, follow up care and Survivorship.* Assesses, plans, and provides patient education and counseling to breast cancer patients providing physical, psychological, and emotional support to patients and their family members.* Collaborates with the multi-disciplinary team to ensure continuity of care from one provider to the next. Tracks the patient closely and ensures that care is smoothly coordinated among disciplines using established tracking system.* Completes the Survivorship Plan of care when the patient completes therapy, facilitates discussion with the patient and sends the Plan of Care to the patient.* Provides counseling for high-risk patients or those with questions (dense breast, positive family history, imaging surveillance et al.) and makes referral to surgical team as needed.* Communicates with physicians and other members of the interdisciplinary team, within Summit Health, as well as with outside providers.* Facilitates scheduling of appointments with other Oncology Departments to ensure timely referral and transition into specialty services.* Provides information on maintaining optimal breast health and prevention of Breast Cancer Recurrence including instruction on the importance of regular visit to their provider for breast exams and imaging as recommended by their provider, maintaining proper nutrition and exercise.* Documents patient visits and communications in Athena. Updates Accession List and Paper lists. Manages and submits statistical data monthly and as requested to Clinical Director.* Covers Imaging Navigation as requested when Patient Navigator is not present. Refers patients for prompt surgical consultation if a diagnostic abnormality is found.* Attends Breast Care Center meetings providing input and suggestions on continuous quality and operational improvement. (Weekly Tumor Board Conferences,

Navigation team meetings, Interdisciplinary meetings, Breast Center staff meetings and others upon request of Clinical Director)

* Develops a familiarity in chemotherapy protocols to support patients during chemotherapy treatment. Obtains either Chemotherapy/Biotherapy Certification or attends ONS Fundamentals Course.

Oncology Navigation:

* Navigates oncology patients across the continuum of care beginning with the diagnosis of cancer to survivorship, coordinating care with staff in Radiology, Surgery, Oncology, Genetic Counseling, Internal medicine and Behavioral Health, and other departments as indicated. Refers to community resources to facilitate timely diagnosis, treatment and survivorship.* Adheres to key Touchpoints for Navigation (attached) including Introduction to Nurse Navigator at time of cancer discussion, Post-operative care planning, follow up care and Survivorship.* Assesses, plans, and provides patient education and counseling to cancer patients providing physical, psychological, and emotional support to patients and their family members.* Collaborates with the multi-disciplinary team to ensure continuity of care from one provider to the next. Tracks the patient closely and ensures that care is smoothly coordinated among disciplines using established tracking system.* Completes the Survivorship Plan of care when the patient completes therapy, facilitates discussion with the patient and distribution of the Plan of Care.* Communicates with physicians and other members of the interdisciplinary team, within Summit Health, as well as with outside providers.* Facilitates scheduling of appointments with surgical staff to ensure timely referral and transition into specialty services.* Provides information on maintaining optimal health and prevention of Cancer Recurrence including instruction on the importance of regular visit to their provider for exams, clinical follow up and imaging as recommended by their provider, maintaining proper nutrition and exercise.* Documents all patient visits and communication in Athena, Accession List and Paper lists. Manages and submits statistical data monthly and as requested to Clinical Director.* Attends Oncology Care Center meetings providing input and suggestions on continuous quality and operational improvement. (Weekly Tumor Board Conferences,

Navigation team meetings, Interdisciplinary meetings, Navigation staff meetings, lectures and programs and others upon request of Clinical Director) including some evening and weekends.

* Develops an expertise in the chemotherapy protocols, related to specific area of navigation, to support patients during chemotherapy treatment.

General Job functions: Oncology Navigation

* Participates in community outreach programs that may include some weekend and evening activities.* Coordinates the availability of patient education materials as well as being familiar with resources available in the community. Orders materials as needed and ensures inventory.* Assists Clinical Program Director with the development and implementation of a comprehensive Oncology Program in multiple areas including meeting National Accreditation Standards, involvement with special programs, survivorship, patient education materials, identification of community resources, marketing, developing and prioritizing workflows.* Participates in Annual Quality Initiatives as outlined by the Oncology Leadership Team.

General Job functions: Breast Navigation

* Participates in community outreach programs. (Cancer Support Group Meetings, in person or virtual, breast cancer patient classes, evening/weekend lectures programs, awareness events). May include some evenings and weekends.* Coordinates the availability of patient education materials as well as being familiar with resources available in the community. Orders materials as needed and ensures inventory.* Assists Clinical Program Director with the development and implementation of a comprehensive Breast Care Program in multiple areas including meeting National Accreditation Standards, involvement with special programs, survivorship, patient education materials, identification of community resources, marketing, developing and prioritizing workflows.* Participates in Annual Quality Initiatives as outlined by the Breast Leadership Team.

Education, Certification, Computer and Training Requirements:

* Bachelor's Degree desired.* Certification as a Breast Nurse Navigator or Oncology Nurse Navigator preferred or ability to obtain within one year.* Registered RN license.* BLS Certification required.* Minimum 5-8 years of experience, preferably with Breast Cancer and/or Oncology patients.* Ability to communicate in English, both orally and in writing.* Ability to use Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.* Ability to use Standard Office Technology including Microsoft Word and Excel.* Experience with an EMR preferred.* Ability to travel between multiple locations, Florham Park, West Orange, Clifton, Berkeley Heights.
