Responsible for the assessment and treatment of patients. Provides patient education and coordinates treatment with agencies and organizations for the delivery of primary health and medical care. Performs diversified clinical duties in support of Obesity Treatment and Optifast Clinics, and the care and treatment of bariatric patients.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Under the general supervision of the Director of the New England Weight Management Institute and Nutritional Services and the Medical Director and within established Catholic Medical Center departmental policies and procedures, the incumbent performs the following functions:

1. Conducts physical, diagnostic, medical history taking, and therapeutic procedures within an outpatient/clinical setting (i.e. bariatric and Optifast Clinics), in a professional manner in accordance with established standards of care and practice.

2. Completes a physical assessment based on interview, examination and available data.

3. Identifies medical and health issues and needs based upon the evaluation.

4. Documents physical findings and patient response on appropriate worksheets and medical record.

5. Collaborates with the physician (as needed) to formulate an appropriate plan of care based on physical exam, patient history and clinical findings.

6. Refers patient and writes orders for psychological assessment, laboratory assessment, neurological, cardiac, respiratory and other testing as indicated by the evaluation.

7. Contacts patients with follow up information on testing, or may at times delegate this to be done by the appropriate Obesity Treatment Staff.

8. Recognizes and reports relevant information that requires urgent attention.

9. Documents data and encounters, including office and phone with the patient in the medical chart.

10. Follows progress of patient care to determine the effectiveness of the treatment plan. Updates and records changes in health status. Modifies treatment to achieve pre-operative or post operative goals.

11. Communicates with referring physician detailing patient progress with his/her obesity treatment program.

12. Identifies patient needs and provides instructions/referrals for patient education.

13. Performs telephone evaluation and triage. Provides medical advice and treatment and documents as appropriate.

14. Writes and calls in prescriptions.

15. Adheres to guidelines for appropriate billing and coding of all patient encounters. Promptly submits billing information.

16. Assists in developing protocols, pathways and treatment plans which improve the delivery of care to patients.

17. Works collaboratively and cooperatively with all members of the multi-disciplinary team.

18. Adheres to infection control procedures and safety control procedures to Universal Precautions.

19. Ensures confidentiality of employee, legal, client/patient, budget and all company matters.

20. Demonstrates courtesy, respect and caring to patients, families, physicians and coworkers and maintains professional rapport with external parties and organizations.

21. Maintains patient charts.

22. Participates in staff meetings and cooperates with physicians and coworkers to achieve departmental objectives. Reports concerns and problems as they arise.

23. Provides and promotes effective and compassionate care for patients and their families.

24. Ensures quality control to maintain overall patient care.

25. Assists with scheduling appointments for coordinating care with other medical providers.

26. Participates in continuing development/professional activities, maintains professional affiliations and licenses and strives to increase nursing skill and knowledge.

27. Participates in appropriate role in disasters, fire drills, fires and other emergency situations.

28. Exercises care in the operation and use of equipment and reference materials. Maintains work area in a clean and organized manner.

29. Performs similar or related duties as assigned or directed.

O Generally Monday through Friday. Work schedule will include day and evening coverage and may include 8-12 hour days based on program and patient need.

Education:

o Masters Degree in Nursing required

o Graduate of an accredited APRN program.

Experience:

o Five years or more as a registered nurse.

Licensure/Certification:

o Current registration in the State of NH as an Advanced Practice Registered Nurse (APRN)

o Basic Life Support (BLS) certification required.

o Advanced Cardiovascular Life Support (ACLS) certification required.
