*Job Summary*We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have experience in various settings including clinics, hospitals, and home community care. This role involves working with patients across different age groups, including geriatrics and pediatrics, to develop and implement effective treatment plans aimed at improving mobility and quality of life. The Physical Therapist will utilize a range of therapeutic techniques, including trigger point therapy, to assist patients in their recovery process.
*Responsibilities** Conduct comprehensive assessments of patients' physical abilities and limitations.* Develop individualized treatment plans based on patient needs and goals.* Implement therapeutic interventions such as exercises, manual therapy, and modalities.* Monitor patient progress and modify treatment plans as necessary.* Provide education to patients and their families regarding treatment options and home exercise programs.* Collaborate with other healthcare professionals to ensure coordinated care for patients.* Assist in discharge planning to facilitate smooth transitions from inpatient to outpatient care.* Maintain accurate documentation of patient evaluations, treatments, and progress notes.
*Requirements** Valid state license to practice as a Physical Therapist.* Experience in acute care settings preferred; familiarity with inpatient rehabilitation is a plus.* Knowledge of geriatric and pediatric physical therapy practices.* Proficiency in discharge planning processes.* Strong understanding of various therapeutic systems and techniques, including trigger point therapy.* Excellent communication skills for effective interaction with patients, families, and healthcare teams.* Ability to work collaboratively in a multidisciplinary environment.

Pay: $45.00 - $47.00 per hour
