JOB SUMMARYThe Restorative - Wound Nurse coordinates, supervises and manages the Restorative Nursing and Wound Programs. Ensures restorative nursing and wound management is provided to the residents as prescribed by the physician and that meets facility requirements and local, state and federal regulations. Provides staff oversight to ensure restorative care, wound care, and preventative skin care needs are implemented as planned.QUALIFICATIONS• Current unrestricted license as a Registered Nurse in Mississippi• Certification in wound care or certified within 1 year or hire or promotion to the position• Completed training in restorative nursing services, or complete training within 90 days of hire or promotion to the position• Prefer experience in healthcare related field with restorative nursing or MDS, and wound care experienceORGANIZATIONAL EXPECTATIONSProvide a positive and professional representation of the organization.Promote a culture of safety through reporting, documentation, prevention, and infection control.Maintain competency and knowledge of current standards of practice, trends, and development in related scope of job role or practice.Participate in ongoing quality improvement activities.Maintain compliance with organizational and regulatory policies and practices.Demonstrate excellent customer service through attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization.Willing to support unit's staffing needs by occasionally performing Staff Nurse duties when needed, adjusting work schedule, staying late or working additional time.PHYSICAL REQUIREMENTSAbility to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.Ability to lift and carry in excess of 50 pounds.ESSENTIAL FUNCTIONS / ASSIGNED TASKS• Provide staff education, consultation, and oversight based on facility policy, planned or identified needs, and regulatory guidance• Conduct daily rounds to direct restorative care partners in the delivery of direct resident care• Perform comprehensive resident assessment and develop a restorative care plan, with measurable objectives and interventions, for each resident who is receiving these services• Update the care plan with progress and/or resident condition changes• Communicate with residents, family members, and other disciplines or healthcare professionals regarding the progress and status of the resident• Review the resident's progress and the continued need for restorative care per facility policy• Provide guidance and assistance as needed for resident transfers, ambulation, range of motion or other restorative needs• Monitor and recommend equipment and supplies needed for the performance of restorative and wound care• Monitor restorative documentation and provide feedback for correction• Assess, identify, manage, and treat specific skin conditions, such as pressure injuries; diabetic, venous, or arterial ulcers; and complicated wounds following facility policy and clinical practice guidelines• Maintain accurate documentation of wound and restorative care provided according to the MDS supportive documentation requirements• Perform accurate assessment and staging of wounds, and provide documentation in the electronic health record• Assess and communicate resident problems, needs, change in condition, or need to alter treatment plan/plan of care to other staff members, physician, and resident representative appropriately and consistently• Assure, maintain and implement the resident care plan as directed, as it relates to wound care, skin integrity, and pressure ulcer prevention• Perform wound assessments, risk assessments, and root cause analysis per facility policy and complete detailed documentation in the medical record• Assist to supervise staff on the unit for proper implementation of interventions related to wound prevention, infection control and maintenance of skin integrity• Educate registered nurses regarding proper identification and staging of wounds• Educate licensed practical nurses in proper identification and treatment of skin issues• Educate non-licensed nursing staff in identifying skin issues and proper preventive measures• Complete staff skills competency checks and audits as assigned• Maintain knowledge of current CMS and MDS guidelines for Restorative Nursing Programs and Pressure Ulcer Prevention, Staging, and Treatment• Work with the QAPI Committee providing wound and restorative program monitoring, data collection and reporting, assisting to identify appropriate performance improvement plans• Assist with admission, discharge, or care of residents as needed• Perform other job duties and tasks as assigned
