What You'll Do:
As a Home Health Physical Therapist, you will carry out our Mission of Love by
providing physical therapy services in accordance with the plan of care. Provide initial and on-going assessment and evaluation of patient response to therapy. Accurately document evaluation and treatments provided. Attend trainings to build understanding and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned.
Our Home Health Physical Therapist is responsible for the following:
Providing services in accordance with accepted standards of practice for physical therapy.Documenting all therapy interventions.Providing initial and ongoing assessment and evaluation of patient/family response to therapy.Developing and revising the plan of care in consultation with the physician and other care team members.Providing therapeutic treatments and evaluation of equipment needs as applicable.Advising and consulting with the family and other agency personnel as applicable.Completing documentation timely for all assessment and evaluation data, treatments and patient's response to therapy intervention.Evaluating outcomes of care and completing the OASIS assessment accurately.Providing discharge planning and evaluation.Supervising physical therapy assistants at least every 30 days and home health aides every 14 days when appropriate.Participating in in-service programs.Participating in IDT and caseload discussionsWhat You Have:
Graduated after successful completion of a physical therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), or successor organizations of CAPTE.Passed an examination for physical therapists approved by the State in which physical therapy services are provided.Valid state driver's license.Motor Vehicle Driving Record satisfactory to the Masonic Village and its insurance providers.Proof of motor vehicle insurance.
