Shift available: M-F 7:30am-4:00pm

Travel Requirements

Must be able to travel to Elizabethtown, PA to attend a 3 week training camp. Training consist of arriving on Mondays, staying in dorm like accommodations, ending on Fridays. Training camp is mandatory for all new hires. Attendance to training camp will be based on hire date; as these trainings are only scheduled on a quarterly basis. Position will begin at the facility upon start date and expectations will be to attend the next training camp. Mileage reimbursement is provided for travel from main facility to Elizabethtown.

Role DescriptionThe BCC MAT Social Worker shall communicate with SCI MAT Social Worker when reentrants OD in community and are returned to SCI for detox to determine eligibility and appropriateness for MAT.The SCI MAT Social Worker shall assist with MAT referrals in the community, including setting up and scheduling appointments for medication and treatment.The BCC and SCI MAT Social Worker shall ensure needs, benefits, and services are obtained in accordance with assigned individuals treatment plans.The BCC and SCI MAT Social Worker shall track and update spreadsheet/database on everyone released from an SCI on a MATThe BCC and SCI MAT Social Worker shall attend weekly Vivitrol Conference Call, and other meetings as needed.MAT Social Workers shall document all contacts in appropriate systems (Captor, Sapphire etc.).MAT Social Workers shall attend all required trainingsMAT Social Workers shall meet with reentrants with positive urine drug screens to assess and refer to treatment.MAT Social Workers shall complete the GPRA as required (Initial, 6 months, discharge).MAT Social Workers shall meet with Reentrants in the SCI, BCC, Parole Offices, and/or Treatment providers as necessary.MAT Social Workers shall communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.MAT Social Workers shall subscribe to the MAT/AOD Treatment framework and policies and procedures of the DOC.MAT Social Workers shall implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment.MAT Social Workers shall perform in accordance with applicable rules, regulations, policies, and procedures under the general supervision of an administrative supervisor.MAT Social Workers shall assess offenders psychosocial strengths, weaknesses, and social work services needs, and eligibility for services.MAT Social Workers shall counsel individuals utilizing various treatment modalities.MAT Social Workers shall discuss assigned individuals psychosocial assessments and family history in multidisciplinary treatment team meetings.Provide support services in areas such as conducting on-site monitoring reviews of long-term living service providers to ensure that consumers are receiving appropriate care that meets State and Federal guidelines.Review written documentation and meet with consumers and stakeholders to evaluate the quality of the services being provided. They will monitor issues related to medical and financial eligibility of the consumer.Skills

Knowledge of professional social work principles, practices, and methods.Knowledge of professional casework principles, practices, and methods.Knowledge of individual and group behavior.Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families.Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.Knowledge of social, economic, health, habilitative, and rehabilitative problems, treatment methods, and resources.Knowledge of the requirements of the Health Insurance Portability and Accountability Act (HIPPa) as well as state confidentiality laws as applied to Social Work practice.Ability to read and interpret rules, regulations, policies, and procedures relating to SoCal work services and programs.Ability to establish and maintain effective working relationships.Ability to communicate effectively in writing.Ability to communicate effectively orally.

Minimum QualificationsBachelor of Social Work from an Accredited Social Work Program or equivalent education and experience.Basic computer skills to include, but not limited to, Microsoft Office Products.
