The Admissions Coordinator reports to the Administrator.
JOB DUTIES:
Aid in developing and executing facility marketing plans.Act as a direct liaison between the facility and hospital discharge planners.Conduct outreach to referral sources.Establish connections to identify unmet healthcare needs in the community and collaborate with the administrator to address them.Complete necessary paperwork to ensure appropriate admissions to the facility.Lead facility admission and census meetings.Manage marketing material supplies and ensure the availability of information packets.Compile daily census reports.Prepare monthly/quarterly reports as instructed.Please handle any questions from hospitals and the community.Evaluate potential admissions:
Confirm payor source.Obtain authorization as required.Verify Medicare eligibility.Assess by level of care to determine placement.
Facilitate admissions process:
Inform nursing and relevant staff of admission.Create resident identification bracelets.Provide charts to the appropriate nurse station.Finalize admission contract with the resident or responsible party.
Maintain Public Relations with referral sources.Sustain post-admission and ongoing internal relations with residents and families.Perform other duties as delegated by the Administrator.
