This is a flexible, autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of our aging population. All the while, you’ll be building meaningful relationships with the residents and their families and the health care providers who are responsible for their care.This position requires travel through the State of New Jersey. Travel between care sites is based on the needs of business / market and required for the role. Expected Travel 75% - 100%

Primary Responsibilities:Obtain and review medical history, conduct assessments, analyze and diagnose conditions, and develop appropriate plan of careIdentify gaps in care and interpret diagnostic test / reports in order to refer appropriatelyIdentify risk factors and help mitigate barriers to accessing care to reduce riskDevelop interventions to assist members in attaining established goals of careEvaluate member’s progress in completion of goals of care and re – assess and assist in care management with membersServe as a key resource on complex and / or critical issues to solve problems and develop innovative solutionsEstablish trusting relationship and maintain communication with the member, family / authorized representative and primary caregiver and specialistsAssist members / families in understanding medical options / interventions and in making informed decisionsClarify member's cultural values that may impact health management / decisionsIdentify prognosis / trajectory of chronic disease that may impact future health decisions and conduct advanced care planning discussions

Knowledge, Skills, and Abilities:Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco usersAbility to lift a 30-pound bag in and out of car and to navigate stairs, as neededAbility to travel 75-100% of the time for field-based workExcellent administrative skills and the ability to utilize technology, such as navigating the Internet on a computerExcellent organizational skills with the ability to multi–task and manage scheduleExcellent collaboration skills and ability to effectively communicate with seniors, their families, their care team, and physiciansYou’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Active, unencumbered APC license to practice in the State of New Jersey (or ability to obtain)Active Nurse Practitioner certification through a national board:

Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:Family Nurse PractitionerAdult Nurse PractitionerGerontology Nurse PractitionerAdult-Gerontology Acute Care Nurse Practitioner

Current, active DEA licensure/prescriptive authority or ability to obtain post-hireAbility to travel 75% - 100% time including overnight travelAccess to reliable transportation, as well as a valid driver’s license, that will enable you to travel to client and/or patient sites within a designated area

Preferred Qualifications:1+ years of experience with hands on, post grad Nurse Practitioner workExperience in geriatric or adult medicineExperience working with complex patient populations, including multiple chronic diseases with diverse psychosocial backgroundsExperience and comfortable working in LTC, home health or in home setting independentlyUnderstanding of Medicare, Medicaid and Health Plan benefit structures beneficialBilingual or multilingual

New Jersey Residents Only:

The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
