The Navigator builds trust, coordinates appointments, and maintains close communication with the patient's care team, addressing individual needs to ensure smooth, efficient access to high-quality cancer care, especially for underserved communities.Responsibilities:Assist with scheduling and completing appointmentsIdentify and help remove barriers to timely treatment, such as transportation or childcareConduct supportive care surveys to assess patients’ symptoms, financial concerns, and care needsOffer support with insurance, appointment reminders, and navigation within the Cancer CenterConnect patients with psychosocial, palliative care, and other supportive servicesScreen for mental health history and refer patients to social work, psychiatry, or palliative care as neededAct as a liaison between oncology and primary care teams for seamless communicationThe Patient Navigator may also participate in other program-related activities as needed.Interested in hearing more?
