2 full-time , compassionate, and dedicated Transition Facilitators to provide case management and outreach to individuals ages 16-25 living in the community, as specified below:Educational and Outreach Specialist
: The main focus of the Transition Facilitator will be conducting outreach in the Framingham community and high schools throughout the MetroWest Area. This person will also provide case management to the college pilot students at Mass Bay Community College. In addition, this individual will assist the Basic Center Program that works with homeless youth between the ages of 12-17. Other responsibilities include providing transportation, health/wellness, budgeting, job/education assistance, and support groups to all young adults of Tempo.Housing Specialist:
The main focus of the Transition Facilitator will be providing case management to all young adults in our TLP/ IL program, along with any young adult with housing-related concerns. Also, this person will be the main liaison between SMOC and Tempo for any housing-related needs/concerns. Other responsibilities include providing transportation, health/wellness, budgeting, job/education assistance, and support groups to all young adults of Tempo.Salary Range: $22.00 - $24.00/hour depending on experience + sign-on bonus (half paid after 30 days of employment and half paid after 90 days of employment; pro-rated for part-time).What You Will Do:Coordinate intake assessments for new participants and schedule weekly case management meetings with assigned participants.Develop, implement, and evaluate service/participation plans in collaboration with participants.Provide individual and group life skills assessment and education/training for participants.Provide employment assistance, career development and apprentice job matching for identified participants.Develop and maintain a broad network of community-based support services, including health, employment, education, counseling, self-help, community service, etc.Provide an appropriate level of monitoring and supervision for program participants, including weekly inspections of each residence to ensure all safety expectations.Complete all documentation as required.Participate in all weekly and required agency staff meetings, required agency trainings, supervision with Program Director and assigned meetings in the community.Assist with development and implementation of appropriate policies and procedures.Ensure compliance with Wayside Youth and Family Support Network policies and procedures.Competencies/Traits of Our Superstars:CharismaticEmpathicDedicationReliabilityTeamworkA bachelor's degree in counseling, psychology, or related field is preferred but not required.Experience working in a community-based setting and/or residential setting with youth is preferred.Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.Bilingual/Bicultural candidates are strongly encouraged to apply.We will train you in:
