The purpose of the IPV is to confirm chronic medical conditions in a patient, provide education on Tapestry's services, and to obtain consent so they can be enrolled in CCM/RPM services (which we bill under the physician lead license number).
The Regional Physician Lead will also anchor a multistate clinical team providing oversight and collaboration to advance practice providers in the nursing home setting. in this cross - functional role you will interface with operations, business developed, compliance, and lead clinical service to support and deliver medical care. You will drive the clinical quality assurance process and ensure compliance to improve services and patient outcomes.
KEY RESPONSIBILITIES:
Initiating Patient Visits:Responsible for establishing the TapestryHealth patient relationship for our product offerings and serving as billing practitioner.Conduct approximately 300 clinical visits onsite or via telehealth per month.Document patient encounters accurately and thoroughly within our platform.Collaborative Oversight:Provide clinical Collaborative Practice Agreements as needed for Advanced Practice Providers (APPs) in your region adhering to regulatory requirement for respective states.Billing responsibility for CCM and RPM services will fall under Regional Physician Lead's license numberClinical Quality:Review patient charts, medical records, and treatment plans to ensure accuracy, completeness, and compliance.Drive quality assurance for Chronic Care Management and Remote Physiological Monitoring with monthly sampling of reports.Provide guidance and feedback to APP team to enhance clinical skills and improve patient outcomes.Serve as final reviewer and billing practitioner for regional CCM reports.In partnership with regional team, track key performance indicators, measure patient outcomes, and identify areas for improvement within the CCM program.
Cross-Functional Collaboration:Acts as lead clinical liaison to facility based medical teams to support the effective adoption of CCM service programs.Partner with business development team to educate our customers corporate clinical leadership and endorse offerings during new client launch or customer success meetings.
Other:Participate in quarterly clinical governance board to review quality issues and/or disciplinary actions needed.Other duties and responsibilities as assigned.
MINIMUM REQUIREMENTS:Qualified MD/DOActive, unrestricted, Medical Licenses in assigned region.Excellent clinical judgment and decision-making skills.Ability to work independently and remotely as part of a multidisciplinary team.Commitment to providing patient centered care and improving health outcomes.Familiarity with electronic health record (EHR) systems and healthcare technology.Proficient with Microsoft Office SuiteAbility to travel within the designated region (when needed).
