This recruitment will be used to fill two current openings - one in Truckee and one in Western Nevada County - and may be used for future openings as they occur.

The Public Health Department promotes the health of the entire community through varied programs ranging from control of communicable diseases and coordination of the response to health emergencies to individual case management to primary prevention of disease and wellness promotion.We are currently seeking two

Public Health Nurses (PHNs)

to be an integral part of our nurse home visiting team! One nurse will be located in Eastern County and work in our Truckee offices, while the other will be based in Western County and based in our Crown Point offices. Public Health's Mother Child Adolescent Health (MCAH) department is dedicated to empowering families through holistic support and fostering strong, healthy relationships. The two PHNs will train in the Parents As Teachers (PAT) curriculum model to deliver evidence-based home visiting services to families with young children. The PHNs will empower and educate parents and families to improve the well-being of the children of our community.

Job Overview: As a Home Visiting PHN, you will work directly with families in their homes, providing support, resources, and guidance tailored to their individual needs. This role emphasizes relational competency, recognizing the importance of building trusting, respectful relationships with families to facilitate positive change.

Establish and maintain trusting relationships with families, fostering a supportive environment that encourages open communication and collaboration.Conduct Home Visits:

Visit families in their homes to assess needs, provide support, and offer resources tailored to their specific circumstances.Facilitate Goal Setting:

Collaborate with families to set achievable goals and develop personalized action plans that empower them to thrive.Provide Resources:

Connect families with relevant community resources, programs, and services that support their growth and well-being.Empower Families:

Use a strengths-based approach to empower families, helping them recognize and build on their strengths and resilience.Monitor Progress:

Regularly assess and document family progress towards their goals, adjusting support strategies as needed.Cultural Competence:

Respect and acknowledge the diverse backgrounds of families, adapting approaches to align with their cultural values and practices.Team Collaboration:

Work collaboratively with a multidisciplinary team to ensure comprehensive support for families, sharing insights and best practices.The ideal candidate will:

Have proven experience in home visiting, family support, or related fields, with a strong emphasis on relational competencies.Exceptional interpersonal skills and the ability to build rapport with diverse families.Knowledge of community resources and social service programs.Strong communication skills, both verbal and written, and will communicate daily with immediate staff, department staff, and outside agencies to gather information, coordinate actions or resolve problems.Commitment to confidentiality and ethical practices.This position is part-time temporary; part-time employees work an average of 20 hours but not more than 29 hours per week, for a maximum of 1000 hours per fiscal year. Temporary employees earn sick leave, but are not eligible for other vacation or holiday pay, medical, dental, or vision benefits, or CalPERS. This position is not open to current CalPERS annuitants.

Sick Leave:

Temporary County employees shall accrue at the rate of one (1) hour of sick leave for each thirty (30) hours worked. Temporary County employees can accrue a maximum of 80 hours of sick leave which may be carried over from one fiscal year to the next.

Medical Benefits:

Temporary employees whose assignments average 30 hours per week or more are eligible to enroll in County-sponsored medical benefits.
