Overview
Todevelop relationships with referral sources and work with internal teams to promote andmaintaincensus. To assistSocialService Department in meeting the needs of the residents and families of the care center.
Every 4th Weekend rotation
Responsibilities/Job Description
Ability to process and complete admissions.
Provides pre-admission counseling,informationand tours.
Clinically evaluates referrals to ensure all needs can be met from a medical perspective
Facilitates the intake processand secures primary care provider
Reviews EPIC for proper documentation and orders
Contacts pharmacy as needed to verify cost of meds
Determinesneed for equipment and supplies and coordinates with internal team
Facilitates PAS Screening and Level II as needed
Coordinates admission process with resident and resident representative, community providers and staff as appropriate
Sets up interpreter services as needed
Assures completion of admission paperwork
Informs residents and resident’s representatives of financial responsibilities, payment options and agency contacts according to the state and federal regulations
Builds andmaintainsrelationships with referral sources andassistswith marketing and outreach activities as appropriate
Maintains internal and external waiting lists as appropriate
Verifiesnecessary payer source(s) as appropriate
Coordinates room change process as appropriate
Ability to promoteprofessional working relationships within Ebenezer/Fairviewand with outside referral sources
Tracks referral sources and other information as requested
Works with internal and campus team to maximize facility occupancy
Refers inquiries toappropriate agenciesas necessary
Conducts outreach through campus and facility events
Participates in Fairview TCN meetings as appropriate
Provides internal trainingregardingadmission process as needed
Ability toassistresidents with varioustasks
Assistsresidents with mail, finances, and MA as appropriate
Notifies residentsand resident representatives ofroom changes and completes necessary documentation
Assistsresidents in ordering personal items (i.e., clothing).
Assistsresidents with securing personal belongings
Advocates for residents
Offers support to residents and families
Ability toassistSocial Service teamwith varioustasks
Oversees care conference rescheduling.
Tracks advance directive andlegal appointment documentation
Oversees resident storage letters
Provides support to socialworkerasidentifiedby supervisor.
Completes and follows up on missing propertyreports
Assistswith follow up on grievances as appropriate
AbilitytoparticipateinVulnerable Adult investigations andability tocomplete necessary documentation and reports.
Qualifications
Requirements:
Vocational/Technical Training orAssociate Degree
Licensed PracticalNurse(LPN) or Registered Nurse (RN)
One or more years experience
Preferred:
Bachelor's Degree:In Medical or Human Service Field
Three or more years experience
Two or more years experience in LTC
