Innovative services offered across Chicagoland and Central Illinois include community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities.Summary:An Intake Coordinator meticulously gathers and verifies important client information, ensures accurate documentation, and provides clients with clear, comprehensive guidance on the processes and services available to them. This role is vital in fostering a welcoming environment and seamless experience for all clients, impacting their journey from the very first interaction.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Essential Responsibilities, include but are not limited to:Responsible for screening potential clients and referrals via telephone by completing an intake screening assessment to ensure criteria and appropriateness are met for programs being inquired about. This includes checking the medi system for active insurance (Medicaid). Offer referrals to outside agencies if criteria or appropriateness is not met.Schedule initial admission appointments for clients using centralized scheduling process and software. Maintain and update spreadsheets for data reporting purposes for tracking referrals, inquiries, scheduled appointments.Oversee the UniteUs referral platform, update the portal with any program changes, respond to referrals within specified time frame. Oversee the Illinois Health Practice Alliance portal regarding client hospitalizations and update the appropriate staff and supervisors of hospitalization. Inform responsible staff of required interventions needed within the specified time frames. This includes checking the electronic health record system to ensure proper documentation has been completed on behalf of the client. Review initial documents with clients. Work closely with the receptionist and office administrator to ensure that signed consents and documentation have been received. Will maintain thorough knowledge of each program offered at Envision Unlimited to ensure accurate information is presented during inquiries. This includes working closely with leadership to understand and have working knowledge of any program updates. Responsible for entering initial/new client information into Envision Unlimited Electronic Health Record. Document appointment details and any additional pertinent information regarding new client in the agency's electronic health record.Responsible for maintaining compliance with DHS, CQL, and any other governmental regulations regarding intake policies and procedures. Maintain accurate and up-to-date client records. Responsible for working with Clinical Supervisors to ensure that the census at each MH program is maintained at capacity. Additionally, the intake coordinator will assist with collecting preliminary information needed for transitions into programs.Responsible for making and maintaining relationships with outside agencies, outreach efforts, preparing and facilitating presentations for hospitals or other agencies to showcase programs and outline referral processes. The intake coordinator will be attending periodic health fairs, marketing events, and other outreach and marketing efforts deemed necessary by the Director of Mental Health and Clinical Services. Attend weekly 1 hour supervision with Assistant Director of Clinical Services to discuss any updates, barriers, concerns to the intake department. Flexible with the growth of the agency and the intake department. Participates in the identification, onboarding and continuous improvement of tools/applications for intake, estimation, resource planning and associated tracking and monitoring. Responsible for any other job duties deemed necessary by Director of Mental Health and Clinical Services. The work environment may include any or all the following: working alone; working with others, verbal contact with others and face-to-face contact.RequirementsEDUCATION, SKILLS, and/or EXPERIENCEMust have a bachelor's degree in counseling, psychology, social work, or related field.Must have experience with verification of benefits (Medicaid and/or private insurance). Previous experience in a customer service or client coordination role is preferred. Proficiency in Spanish language at a professional level preferred. Must have strong verbal and written communication skills; as this position requires face-to-face and telephone interaction with various levels of management, clients, and outside agencies. Knowledge of Microsoft Excel Strong ethical standards and ability to maintain client confidentiality. Strong organizational and multitasking abilities. Ability to work independently and as part of a team. Must be available to work in-office 5 days a week, this is not a hybrid or remote position.
