Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB

SUMMARY:

The Certified Nurse Midwife (CNM) is responsible for providing and managing ambulatory and outpatient quality health care services to patients of CommuniCare+OLE. The CNM directly reports to the Medical Director, Women's Health Services. The CNM coordinates prenatal services and ensures strong communication between prenatal providers, CPSP providers and with the community Obstetricians.

KNOWLEDGE OF WORK

Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines

Knowledge of Electronic Health Record and associated computer programs

Ability to collaborate with mutual providers of treatment in primary care setting

Ability to prioritize and multi-task and have strong time management skills

DUTIES AND RESPONSIBILITIES

Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population. Consult with staff physicians and Family Medicine providers as needed. Refers patients to specialists and to relevant patient care resources as appropriate

Completes documentation of medical services within the EMR and attends to administrative patient care tasks in a timely fashion and in accordance with CommuniCare+OLE policies and standards

Provides prenatal and postpartum care for low - moderate risk pregnancies. Consultation with Obstetrician for co-management of specified medical complications encountered during pregnancy

Refers all eligible clients to CPSP and Sweet Success services

Collaborates with CDE in managing Care Plans for diabetes in pregnancy

Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within their scope of practice

Collaborates with WHS - Manager and the AMD -WHS to coordinate pregnancy and related women's health services to ensure highest level quality of care and access to care with for the provision of Women's Health and Reproductive services at CommuniCare+OLE. Works with AMD -WHS and Clinic Director of QI to develop, implement and promote the QI initiatives related to women's health and pregnancy care

Collaborates with the AMD - Women's Health Services and Manager to coordinates Perinatal Clinical Services at CommuniCare+OLE

Participates in Prenatal Case Conference with St Helena Women's Group and Queen of the Valley Medical Associates

Collaborates with AMD -WHS to develop and implement Clinical Protocols and Guidelines for Prenatal/Postpartum care at CommuniCare+OLE

Participates in recruitment and orientation of Medical Providers and nursing staff

Directs and coordinates the patient care activities of nursing and support staff as require

Follows established departmental policies, procedures, and objectives. Continuous quality improvement objectives, and safety, environmental, and/or infection control standard

As appropriate to the position, participates in specific health promotion, education and/or prevention programs

Performs other duties as assigned

EDUCATION, EXPERIENCE, TRAINING State of California CNM License. Furnishing License and DEA Certificate. Proficiency in Spanish is required. Bilingual in English/Spanish preferred. Must certify and remain current in CPR certification. Experience in a community healthcare center preferred.
