Physical Therapist (PT)

The Physical Therapist for PACE is responsible to assisting the physical therapy needs of each participant which includes initial assessments for potential participants, annual assessments, and any time there is a change of condition. This position collaborates with the interdisciplinary team, participants, and their family members to create, update and implement the plan of care (POC) that is individualized for each participant's needs to improve their quality of life. This position will also be responsible for providing clinical education and training to PACE staff and volunteers to maximize safety and functional abilities of participants while they perform activities of daily living (ADL's).

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 163 PACE programs, serving more than 72,000 participants in 32 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:Graduate from a CAPTE accredited Physical Therapy program.Currently licensed as a PT by the state of Colorado and maintained throughout employment.1 year of PT experience required; 2 to 3 years of home health or other home-based PT experience preferred.Valid Colorado driver's license, unlimited access to reliable transportation, and adequate automobile insurance is maintained throughout employment.Able to work in a variety of environments.Adequate computer literacy in MS Word, Excel, Access, OutlookCurrent CPR certification is maintained throughout employment.Excellent interpersonal and communication skills.Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.Effectively work with and build relationships with others.What We Offer:
