A rehabilitation provider since 1983, Tx:Team brings specialized Geriatric Physical and Occupational Therapy Services to the Somerby Peachtree City community.

Community is nestled in beautiful Peachtree City, GA and less than an hour south of Atlanta, GA.The beautiful campus includes three living options for seniors: Independent Living, Assisted Living, and Memory Care. At Somerby Peachtree City, the mission is to inspire and nurture successful aging and our multi-disciplinary rehabilitation team will play an important role in keeping residents as independent and healthy as possible.Somerby Peachtree City has a state of the art therapy gym with access to fitness equipment, pool, and private treatment rooms; providing the optimal atmosphere for our treatment model for therapy services which includes 1:1 patient care and a therapy-led onsite fitness program.Patient care schedule is Monday - Friday with no evenings, weekends, or holidays.Opportunity to grow outpatients from a variety of providers as well as a senior population that is eager and in need of therapy. care with a highly trained clinicianIs someone who enjoys participating in or leading community and education eventsIs someone who values a lifelong relationship with patients and who can provide therapy services across the lifespanIs someone who is focused on customer service excellenceThe requirements:
Degree from a CAPTE accredited schoolCurrent state licensure or eligibility in disciplineHas minimum of two (2) years geriatric outpatient clinical experienceExperience with aquatics, balance/vestibular, and neuroLSVT BIG Certification a plusDisplays flexibility and strong time management and communication skillsDemonstrates confidence, initiative, and creativity to build caseload within a communityProficient with EMR (Electronic Medical Records)Basic Life Support certification through the American Heart Association, or ability to obtainThe clinical responsibilities:
Deliver One on One quality physical therapy services according to treatment plans for patients.Document progress toward identified objectives.Provide accurate and timely documentation for current patient caseload within EMR.Create a positive work environment to promote teamwork and communication.Conduct patient and family education.The benefits:
