Overview
We are looking for a
Statewide Travel Nurse Practitioner
to join our Advanced Illness Management team. This position will directly report to the AVP of Advanced Illness Management (AIM) and is responsible for facilitating and providing advanced illness management and palliative care services to Empatia patients.Provide individualized treatment plans and comprehensive care from the comfort of patients' home: including providing diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director or Primary Care physician.Role requires up to 80-90% statewide travel.
Requires regular travel between palliative branches throughout the state to provide patient care and other essential responsibilities.
May potentially be some occasional regional travel if needed (less than 10%).Flex or alternative schedules available.About You
Minimum of Master of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law, i.e. DNP (Doctorate of Nursing)2+ years Nurse Practitioner experienceCurrent license as a registered nurse and equivalent as required by state laws for Nurse Practitioner licensure.Active and unrestricted Nurse Practitioner certification/license.Multi state licensure is desirable for Travel Nurse Practitioner role or willing to obtain additional state licensure (company paid) for those interested in regional travel opportunities (e.g. assist with new site launches)Practice experience with Part B documentation and codingMust have a collaborative agreement in place with a supervising physician agreeable to supervise work in palliative care program or must agree to enter a collaborative agreement with Palliative Medical Director, as required by state law.Part B Provider certificationFederal DEA licenseState DEA license or state specific prescribing authority (if required)ACHPN - Certification in Hospice and Palliative Care PreferredNurse Practitioner specialty in Family or Adult Medicine, Acute Care preferredTravel: Up to 80-90% (by car)We Offer
