Role:

The Staff Nurse - RN provides nursing services at the top of the incumbent's license to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include but are not limited to triage; access; care coordination; and nursing support of clinical sessions.

Responsibilities:Performs all nursing duties in accordance with Massachusetts Nursing Practice Act, while adhering to Harbor Health policies and procedures.Assists providers and Care Team members in caring for patients.Performs routine nursing care to patients, while utilizing age specific guidelines and standard precaution practices at all times. Performs comprehensive assessment of patients health status and triage based on urgency of symptoms, in person and via telephone.Performs patient teaching for all patients and families.Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice to holistically care for the patient.Provides patient education on disease prevention, medications and treatment plans and self-care techniques.Acts as liaison between the patient, the care team and/or provider.Provides health coaching and teaching for common conditions (i.e. asthma, hypertension, diabetes) through individual nurse visits, group visits and co-visits with other disciplines () as appropriate.Helps patients stay up to date with clinical care guidelines, immunizations and health maintenance screenings.Administers medications and vaccinations to as ordered by provider or by following standing orders policy.Assesses information about the patient's physiological, psychological, sociological, and spiritual status.Assists in the orientation, evaluation, and training of new staff to the department.Promotes health and wellness through education and preventative care.Counsels patients on lifestyle modifications as needed.Documents patient care activities, assessments, treatment plans and interventions in EHR, ensuring accurate and timely documentation of patient encounters.Supports patients in navigating the healthcare system and understanding their patient rights and options.Communicates with staff, patients, and families demonstrating excellent written and verbal skills.Consistently seeks educational tools to broaden knowledge base of various age groups, diseases, and best practices.Demonstrates knowledge of medical equipment and appropriate usage.Maintains knowledge of all medications, reactions, documentation, and dosage requirements.Conducts procedures according to policy while ensuring patient safety.Certifies that emergency equipment in clinical area is appropriately maintained and in working order.Follows HHSI policy when disposing of biohazardous materials, labeling and handling all materials.Participates in team, department, and clinic meetings to learn, support process improvement, and contribute to care plans as appropriate.Provides cross coverage as needed (Primary, Women's Health and Pediatrics.)Requirements:

Successful completion of a nursing program (RN), BSN preferredCurrent MA RN License, current BLS (Basic Life Support) certificationExperience in a community health setting preferredAmbulatory care or health insurance environment experience preferredBasic computer literacy, knowledge and experience with Microsoft Office 365 including Word, Excel, Outlook and Teams, knowledge of Electronic Medical Record (EMR), EPIC/OCHIN EMR preferred.Excellent customer service skillsExcellent verbal and written communications skillsAbility to effectively work within both a team setting and independentlyAbility to work effectively with economically, racially and linguistically diverse customersBilingual a plus, English and Vietnamese/Spanish/Portuguese or Haitian Creole
