Job Description

Purpose:

Under the direction of the Director of Nursing/Nursing Education and in collaboration with the Director of Nursing/Ambulatory Care, the Assistant Director of Nursing/Certified Diabetic Educator will develop, plan, and implement educational activities and teaching related to diabetes care and management on an ongoing basis.Demonstrates leadership that is self-directing, creative and innovative with an ability to evaluate self objectively and function effectively in stressful situations. The nurse will be primarily responsible for the coordinating the Diabetic Centers of Excellence Journey.

Specific Duties and Responsibilities:• Serves as a consultant and resource to the clinical team and patients in diabetes management• Functions as a clinical role model, coordinating delivery of nursing care to diabetic patients by assessing diabetes health care status, educational needs and support• Supports diabetes education in the ambulatory care and acute care settings• Utilizes and incorporates evidence-based practices in diabetes education• Collaborates with the treatment team and makes recommendations and or referrals, as indicated• Ensures that diabetic educational materials are current and readily available in various languages• Collaborates with clinical team in improving clinical outcomes with implementation and adherence to treat to target (T2T) for diabetic care• Communicates with the primary care provider for medication management• Assesses knowledge base of patients, providing teaching on diabetes self-management and facilitates patient compliance by incorporating patient's perception and goal(s) in plan of care• Oversees educational offerings and trainings of newly diagnosed diabetics in person or by chart review; providing recommendations consistent with ADA guidelines• Shares clinical expertise with the clinical staff on new or unfamiliar technology, procedures, education, trends, and practices• Organizes and participates in community outreach activities focused on diabetes management and care• Networks with internal (NYC Health + Hospital) and external (nationwide) Diabetic Educators• Be an active participating member of the American Association of Diabetic Educators• Acts as liaison by providing resources or links for needs identified through social determinants of health (SDOH) screening• Assesses knowledge base of the nursing team and makes recommendations for improvement• Facilitates and or conducts individual or diabetic group teaching in the context of support groups, as needed.• Engages in the Patient Centered Medical Home (PCMH) model collaborating with the patient and the care team focusing on the goals and objectives for the promotion of optimum health, preventive care and self-management• Acts as a liaison between patient, family members and doctor.• Uses group process to identify, analyze, and resolve care problems.• Develops and maintains effective relationship with administration, nursing staff, allied health professionals and other support services to facilitate the delivery of diabetes nursing care.• Develops, implements, and evaluates educational activities based on learning needs survey of the team.• Participates in accreditation and PI activities• Collects and maintains data on diabetes related metrics and performance• Actively participates in scheduled supervisory meetings/classes• Incorporates diversity, equity and inclusion during all interactions

Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).Special Note:For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
