Acentra is looking for a Clinical Evaluator to join our growing team.Job Summary:As a Clinical Evaluator PASRR for Preadmission Screening and Resident Review, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in the state of Kansas. This Clinical Evaluator position is a part-time role. Your role involves providing essential evaluations to enhance patient well-being and improve their quality of life. The purpose of this position is to complete face to face, clinical assessments of individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC) admitting into nursing homes. These preadmission screening and resident review (PASRR) Level II evaluations typically occur in nursing home or hospital settings. We are looking to hire Clinical Evaluators in the following Kansas cities: Wichita, Overland Park, Kansas City, Olathe, Topeka, Lawrence, and Shawnee.Job Responsibilities:Ensure accurate evaluations and recommendations through accurate clinical assessment of the patient, relevant records, and collateral sources of individuals.Recommend the appropriate level of care and service needs based on the patient’s physical behavioral, and psychological functioning.Complete assessments on time and within scope of practice to meet all contractual requirements.Manage/Supervise the completion of Screens for Continued Stay for individuals with several and persistent mental illness residing in nursing facilities for mental health.Strong typing and computer skills required with proficiency in Microsoft Word and Outlook.Ensure compliance with program workflows.Ability to complete person-centered assessments with meaningful and appropriate recommendations for level of care.Excellent clinical experience and knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in Kansas.Determine a clinical diagnosis accounting for psychological and behavioral issues.Ensure that compliance with necessary workflow, including re-review of records based upon input from the psychiatrist.Knowledge of standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other disorders typically requiring long term care services and supports.Work collaboratively with healthcare professionals, individuals and families, and other community supports.Communicate effectively with individuals and groups through spoken, written, and electronic media.Ability to attend to detail, effectively prioritize, and execute tasks in a timely manner.The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.Required Qualifications/Experience:Licensed to practice in the State of Kansas as a Licensed Master’s level Psychologist, Licensed Clinical Psychotherapist, Licensed Marriage and Family Therapist, Licensed Clinical Marriage and Family Therapist, Licensed Professional Counselor, Licensed Clinical Professional Counselor or Licensed Social Worker or Registered Nurse with A specialty in Psychiatric Nursing).Graduation from an accredited College with relevant Degree Program.Experience with geriatric population is preferred.Minimum of (1) year of experience in working with individuals with intellectual disability or related conditions.Preferred Qualifications/Experience:Proficiency in medical record abstraction.In-depth knowledge of medical record organization, medical terminology, and disease processes.Strong clinical assessment capabilities and critical thinking skills.Exceptional verbal and written communication abilities.Effective teamwork and collaboration skills.Flexibility and excellent organizational abilities.Proficiency in Microsoft Office and internet/web navigation.Familiarity with Federal Regulation 42 CFR, Chapter 483.Comprehensive understanding of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM).Knowledge of standards of practice for determining Levels of Care.Expertise in standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and conditions typically requiring long-term care services and support.Why usWe are a team of experienced and caring leaders, clinicians, pioneering technologists, and industry professionals who come together to redefine expectations for the healthcare industry.
