Residency Responsibilities:
Job Summary : This 12-month post-graduate Family Nurse Practitioner (FNP) residency provides additional skills training as well as independent mentorship experience in a community health center. The intensive clinical program prepares Family Nurse Practitioners to meet the needs of the culturally diverse and clinically complex patients who are typical of a community health center practice. Following the completion of the residency, resident agrees to work a second year as a full-time nurse practitioner at the health center.
Work in a full-service Patient-Centered Medical Home, side by side with clinicians experienced in treating a diverse patient population, and who understand the importance of population health management
Have close ties to and participate in programs with the UMass Chan Medical School Tan Chingfen Graduate School of Nursing
Within the health centers’ family practice model, Family Nurse Practitioner will be exposed to a wide variety of health care services including chronic disease management, medication-assisted treatment, integrated behavioral health care, reproductive health, LGBTQ+ care, global health, and HIV services
Engage in a variety of programs and services that enhance care delivery. Quality improvement initiatives, programs addressing the social determinants of health, and group visits are just a few of the many opportunities for Family Nurse Practitioners to hone their skills and find their own path in the clinical world. The health center also uses a variety of technology tools, including a patient portal, an electronic health record and other clinical technology to enhance its health care services
Develop and maintain panel of patients to facilitate continuity of care. The hallmarks of this residency include distance learning opportunities for the residents along with the peers from other residencies programs ,and with skilled preceptors for clinical expertise and mentorship during clinical sessions
Residents are employees of their residency employment site and will comply with all patient care policies
Clinician Responsibilities:
Deliver direct Primary Care to assigned patients. Develop therapeutic plans of care for prevention, management and maintenance of optimum health for individuals
Provide care related to prenatal/postnatal services and care for uncomplicated pregnancies in conjunction with physician partner as delineated and approved for in request for clinical privileges
Provide care for Acute and Chronic Illness and Urgent Care services that includes assessment, diagnosis, treatment and monitoring of illness, and preventive services to children and adolescent populations within the assigned site/location as assigned by the designated clinical supervisor
Consult with physician partner and APP preceptors and/or refer to subspecialty care patients with more complex/dynamic problems.
Make referrals to health care professionals, specialists, other clinics and community agencies
Maintain and update Electronic Health Records (EHR) in a systematic format and complete required documentation within the specified amount of time expected by the assigned manager/director. Ensure that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment
Education and Experience :
Master’s or Doctoral Degree and board certification as a Family Nurse Practitioner.
Active, Unrestricted Nurse Practitioner License in Massachusetts.
The Health Center requires all employees to have the most recent COVID19 Booster and the yearly Flu Vaccine.
