In this crucial role, you will provide strategic and operational oversight of our case management and social work service programs within our hospital and medical foundation. Your leadership will be vital in delivering high-quality, cost-effective, and integrated patient care processes while optimizing our revenue cycle.As the Director, you will lead a multidisciplinary team, establish care coordination standards, and create strong partnerships with community organizations. You will ensure compliance with regulatory and accreditation requirements, drive patient and family engagement initiatives, and manage budget planning for care coordination services.The ideal candidate is a licensed healthcare professional with at least 5 years of management experience in case management and/or utilization management within an acute care hospital. A Bachelor's degree is required, along with relevant healthcare or management certifications.
