Actalent is seeking a dedicated and compassionate Registered Nurse Case Manager to join our team (Open to Entry Level and Experienced!!)
The ideal candidate will have a strong commitment to public and community health, with the ability to work with individuals from diverse
ethnic and socioeconomic backgrounds.
This position will sit remote 2 days a week AFTER 90-day training period

Responsibilities:
Conduct intake assessments with patients and families to understand their needs and develop individualized treatment plans.
Ensure that families feel sufficiently equipped and supported throughout the treatment process.
Develop and adjust treatment protocols as needed, considering physical, financial, and other constraints.
Coordinate care plans and work with other healthcare providers to ensure that patients receive the necessary care.
Monitor patients’ recovery progress and update care plans as necessary.
Maintain accurate and up-to-date patient records and documentation.
Provide education and support to patients and their families regarding health conditions and treatment plans.
Collaborate with medical staff and other team members to provide high-quality care.
Ability to work with individuals from a variety of ethnic and socioeconomic backgrounds
