Job Title: Pharmacist Job Specialty: Pharmacy Job Duration: Full-Time Shift: Days Guaranteed Hours: Not Specified Experience: Not Specified License: Unrestricted license as a registered pharmacist in any state Certifications: - Basic Life Support for Healthcare Providers (CPR and AED, may be obtained within 1 year of hire) - Completion of a nationally recognized pharmacy residency program preferred Must-Have: - Pharm.D. degree (Candidates licensed prior to 2003 with a Bachelor of Pharmacy degree will be considered with 3 years of experience in hospital and/or ambulatory clinic pharmacy) Job Description: - Function within the pharmacy department to ensure efficacious, safe, and cost-effective drug therapy and appropriate medication outcomes. - Prepare and distribute medications according to Federal and State law. - Provide patient counseling and education on medications. - Advise medical, nursing, and staff on appropriate drug therapy. - Select and manage an appropriate drug inventory. - Formulate and participate in quality improvement activities within the department. - Ensure confidentiality and professionalism in all interactions. - Utilize independent professional judgment to evaluate each prescription for appropriateness and safety. - Conduct drug utilization evaluations and decision-making for policy and procedural formulation. - Review all prescriptions for legality, completeness, contraindications, interactions, allergies, adverse reactions, and lab abnormalities. - Provide clinical consultation and intervention documentation. - Enter prescriptions into pharmacy software and verify orders for accuracy and compliance. - Advise patients on medication instructions and expected outcomes. - Perform chart reviews for prescription refills and medication therapy management. - Direct pharmacy support personnel and serve as a role model and educator for pharmacy students and technicians. - Participate in clinical and administrative activities, including quality control and improvement. - Provide health promotion and disease prevention activities related to drug use and preventative drug therapy.
