GOBHI has an immediate opening for a full-time Clinical Care Coordinator and we are flexible on the home office location within the State of Oregon. This position will be based out of the employee's home. We may be advertising for this position in several cities so please only apply to one job posting.
The Clinical Care Coordinator will provide high quality, integrated, trauma informed and culturally competent care coordination services to members who have complex behavioral health care needs. These may include medical, substance use and any other comorbidity from dental to dementia.
The Clinical Care Coordinator is a key position with a primary purpose to make sure members' needs are met by developing plans of care. This position works directly with community based providers of patient care, including; hospitals, Community Mental Health Providers (CMHP), allied agencies, residential and primary care clinicians in addition to peer providers and attachment figures. The position also cross covers in the Utilization Management (UM) role and necessitates the ability to work after hours and weekends on occasion.
Responsibilities include:Document and present coherent clinical assessments and case formulations. These assessments will include all dimensions of health risks, strengths and potential needs, including life skills, physical and mental health, substance use, social support and attachment dimensions.Develop individualized plans of care.Teach and provide telephonic support for members and those around them.Use clinical and system of care expertise to provide access to culturally and linguistically appropriate services.Coordinate with providers at all levels of care.Coordinate with member, treatment team, and substance use disorder service providers such as Detox, Residential, Outpatient and Medication Assisted Treatment programs to enable access to appropriate levels of SUD care at the time the member is ready.Document the continuous evaluation of the delivery of interventions designed to meet the goals of the care plan.Facilitate communication between members and their support systems.Participate in interdisciplinary or multidisciplinary team meetings associated with member care.Provide technical assistance and training to community partners regarding our programs and systems of care.Perform cross coverage when required, fulfilling all duties and services of the GOBHI Utilization Management Integrated Services Coordinator.*Resume and Cover Letter are required to be considered for this position.*
Requirements
What is needed to succeed in this role:Experience - 5 years' experience in mental health, knowledge of current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) criteria for mental health and substance dependence/abuse diagnoses, ASAM (American Society of Addiction Medicine) criteria for alcohol and/or drug dependence treatment and mental health.Experience- working with programs such as EASA, IIBHT, WRAP, or experience coordinating between levels of care with hospital or DHS.Ability to conduct psychosocial assessments, including history of mental and physical disabilities, alcohol and drug use, past mental health and criminal justice contacts, family, and social relationships, and level of functioning, mental status examination and formulate multiaxial DSM impressions and mental health risk assessments and intervene accordingly, if indicated.Ability to determine the quality of assessments, treatment and discharge plans, as well as patient outcomes, synthesize complex medical and behavioral health diagnostic information to assist providers and other care coordinators in treatment planning and write and supervise an individual plan of care.Communication skills - ability to present, recommend, and listen at a high functioning level.Technology Savvy - proficient in Microsoft Office software and/or Google Suites.Strong planning and organizational skills.Positive attitude - ability to approach problems in a positive manner.Education - Master's degree required with major study in social work, psychology, sociology, or a closed allied field. Licensed Clinical Social Worker or other Oregon licensed healthcare profession required or equivalent combination of education and experience.Bilingual in Spanish preferred.What we offer:
