POTENTIAL HYBRID REMOTE, SOME EVENING WORK JOB SUMMARY: The Resource Navigator develops and implements strategies to assist older adults to access their needs following referral to Center For Hope or the Elder Justice Multidisciplinary team or partner following an incident(s) of abuse or victimization. This role requires knowledge of community resources and continually updating awareness, new and diverse sources of support for older adults within LifeBridge Health as well as the Baltimore Metropolitan region. This will require research and investigation of organizations regarding their offerings and procedures to evaluate appropriate referrals for older adults served through the Elder Justice Program. The Resource Navigator will meet with older adults virtually and in person at Center for Hope and other agencies, depending on the client’s needs, and will participate in the Multidisciplinary Team and other meetings as directed.
