GENERAL SUMMARY:
Summary: Works independently and with autonomous collaboration with physician to provide complete physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The Nurse Practitioner works within a collaborative model to provide quality care and to improve patient outcomes by strengthening continuity of care in all settings for patients, families, and staff in both the hospital and physician practice settings. The Nurse Practitioner (NP) remains responsible for the patient throughout the course of the treatment, provides the physical examination, the diagnosis, and the health care plan, determines when the patient has recovered, and releases the patient. The NP may refer the patient to physicians for consultation or to specialized health resources for treatment but retains responsibility for clinical management as well as any subsequent medication.
ESSENTIAL FUNCTIONS & ACCOUNTABILITIES:
1. Accurately performs history and physicals for patients. Utilizes sound clinical judgment based on best practice when implementing the therapeutic regimen. 2. Documents all care provided, including pharmacological dispensing, in an accurate manner. 3. Completes patient discharge planning and education; writes appropriate orders and prescriptions. Performs patient education and counseling to promote wellness. 4. Evaluates patient's progress toward attaining expected outcomes. Evaluates effectiveness of interventions in relation to outcomes. Documents patient's response to interventions. 5. Maintains accurate, legible, and confidential medical records in accordance with federal and state laws. 6. All other duties as assigned, within the scope and range of job responsibilities.
REQUIRED MINIMUM EDUCATION AND CREDENTIAL:
Education Master Degree Certificate Program - Nurse Practitioner Credential Delaware State Certification/License Advanced Practice RN - Controlled Substances Registration Basic Life Support
EXPERIENCE:
Required: Two years' experience as an Nurse Practitioner in a physician practice setting and/or related nursing experience Preferred: Three (3) years Nurse Practitioner experience in specific area of practice (i.e. Oncology office).
