Job Summary:
The Transplant NP practices as an independent care provider in collaboration with the transplant physicians, including the Chief of Transplant, primarily on the inpatient Renal & Transplant Unit and the Transplant Short Stay Unit and out-patient settings. Her/his practice is characterized by comprehensive health assessments and management of patients with acute and/or chronic renal disease. The NP shall have clinical responsibilities inclusive of transplant candidates, living donors, post transplant patients and patients enrolled in research trials. Obtains patient histories and performs physical examinations. Writes admitting orders, routine orders and daily progress notes as indicated. Acts as liaison between Saint Barnabas and all referring dialysis centers, community agencies and referring physicians. Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department. Uses an understanding of the principles of growth and development to assess each patient s age specific needs and provides age specific treatment.4*10hr shifts weekly with-rotation to weekends.
Essential Functions:
+ Obtains health histories, examines and treats potential transplant candidates, transplant recipients and living donors primarily on the in-patient Renal & Transplant Unit and as requested in the pre and post transplant outpatient settings under the direction of the collaborating transplant physician. Completes admission and discharge summaries and communicates with patient s primary transplant nephrologist in a timely manner.
+ Provides consultation, education, and referral services for transplant candidates, transplant recipients and living donors. This may include appropriate referrals to endocrinology (i.e. steroid induced diabetes); surgery (i.e. lymphocele drainage); cardiology (i.e. clearance for transplant candidate); urology (i.e. stent insertion/removal) etc.
+ Performs clinical duties related to scope of practice including such things as staple removal; wound care; adjustment of immunosuppression; review of recorded BP and adjustment of anti-hypertensives accordingly; monitoring weight; adjusting diuretics/monitoring electrolytes etc.
+ Acts as a liason between transplant coordinators, transplant physicians, transplant consultants and ancillary staff in providing continuity of care to patients and families.
+ Participates in the development of appropriate clinical protocols and related policies and procedures.
+ Participates in patient care conferences including weekly Case Review meetings, Transplant Grand Rounds; Transplant Research Meeting; Transplant Candidate Review Board and Journal Club.
Job Requirements:
+ Certified Nurse Practitioner licensed in the state of New Jersey. Registered Nurse in the state of New Jersey. Certified Clinical Transplant Coordinator (CCTC) certification preferred.
+ Experience with ESRD population and /or transplantation is preferred.
+ Must seek and maintain current certification in the following:
+ Basic Life Support (BLS)
+ Successful completion of all Orientation Programs.
