Position Overview:
Will be providing Client services by monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients.What to expect day-to-day as a Mobile Health Clinician:Each day, will be paired with a driver to patients' homes to provide care and monitoring.While at a patient's home, will engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare systemThroughout the treatment process, maintain patient medical records and documentation consistent with state regulations, client standards, and policy through the HealthPoint and Athena EMR systemWill be practicing under direction with synchronous telemedicine support
Responsibilities:
Act as a conduit for the medical treatment of the Health Plan Partnerships patient populationNavigate conversations related to management of health, well-being, medication management, and goals of careProvide supervised, directed treatment of common medical diagnosesParticipate in continuing education as required by the state where you reside and the Client training teamAdhere to clinical and safety standards, protocols, and performance metricsProvide care with cultural sensitivity and compassionLead your practice with a patient-centered approachOther related duties as deemed necessary
Qualifications:
Minimum of two years of experience as a Licensed Practical NurseActive and current American Heart Association BLS RequiredEmergency Department, Urgent Care, Internal Medicine, Hospital - Critical Care, Family Practice or other relevant areas experience preferredCurrent unrestricted license NY State LPNClinical work experience with pediatrics requiredBlood Draw/Phlebotomy Certification Required in addition to on the job experience drawing blood.Fluent in Spanish language preferred
