Job Summary

The Licensed Practical Nurse (LPN) Clinical Care Leader (CCL), Long-Term Care (LTC) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized care in the long-term care setting while under the supervision of a registered nurse, advanced practice provider, or physician.

This position's main duties include completing, updating and maintaining the Schedule for our Nurses and CNAs, making and maintaining up to date schedules for appointments and Physician visits for our Residents, updating and ensuring accuracy of Timecards for the nursing department, and leading orientation training for New Hires in the nursing department. Collaborates with other inter-professional colleagues, to plan, implement, and evaluate care. Documents patient/resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality patient/resident care, collaborates with inter-professional team members, effectively delegates to and supervises unlicensed personnel, assumes overall nursing accountability for a patient/resident assignment, and serves in leadership roles on department/organizational task forces and committees. Safeguards residents' property. Maintains residents' self-esteem, privacy and confidentiality of personal information. Provides residents with opportunities for independence and choice consistent with their care plans.

Assists to maintain safe, pleasant, orderly resident living areas. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area or unit.

Utilizes communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients/residents, and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations. Performs coordination of care, patient/resident assessment, patient/resident education, triage, and various other nursing interventions.

Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility. Specific services/positions may have additional education, training or experience requirements.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.
