Facilitates a timely, integrated and coordinated approach to the care of acute patients in the home setting (Hospital at Home). Uses a multidisciplinary approach and acts as a liaison between the patient, staff and acute care hospitalist as well as ancillary areas of the hospital system. Identifies learning needs and content for educational programs. Develops, presents, coordinates and evaluates educational activities. Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.
Requirements
Education: Bachelors degree in nursing required, Masters preferred.Experience:Minimum of 3 years acute care clinical experience.Certification: American Heart Association Provider Basic Life Support (BLS)within 30 days of hire.License: Currently licensed or eligible for RN Licensure in the State of Florida.Other: Strong communication and interpersonal skills with the ability to flex and adapt to evolving environment and role. Computer systems, telephone/video, and customer service skills required.
