OverviewThe Nurse Practitioner (NP) is a mid-level health care provider, who works collaboratively within a multi-disciplinary health team and is responsible for providing direct comprehensive care to patients and supervision of ancillary staff. The NP educates the patient, family/significant other by promoting wellness, preventing health care problems, maintaining current health status, and intervening in acute or chronic illness. In collaboration with an attending endocrinologist, is responsible for the coordination and direct care of patients within the specialty of Endocrinology and Metabolism in the outpatient setting. ResponsibilitiesAssist in identifying and caring for individuals with diabetes mellitus. This will include type 1 and type 2 diabetes as well as patients on insulin pumps. This position will allow growth into other endocrinology disorders such as thyroid disease, and bone health.Identify at-risk patients, provide evidence-based nutrition and disease specific educational interventions, coordinate care with team members, and prevent avoidable utilization and adverse outcomes.Interprets continuous glucose monitor and pump readings.Provide teaching on new insulin administration, meter teaching, insulin pump malfunctions/reviewProvides diabetes counseling and education to patients and families as well as plan and put together dietary meal plans with patients based on their medical conditions, restrictions and needs.Responsible for utilizing professional skills and experience to educate patients with Type 2 Diabetes and healthcare professionals on the correct administration of injectable prescription medicinesMonitor patient progress toward acceptable glucose and A1C levels, exercising initiative to adjust medications that include insulin and other diabetes management pharmaceuticals.Collect and document assessment data, educational plan, educational services and evaluation results in compliance with ADA or NCQA Recognized Program process.All other duties as requested. QualificationsEducation:Master's Degree Program In Nursing / Nurse Practitioner.Experience:Minimum one (1) year as nurse practitioner experience required.Experience in endocrinology.Experience in Diabetes Management and Insulin Pumps.Knowledge and Skills:Strong computer skills, including Word, Excel, and PowerPoint.Familiarity with EMR software preferred.Strong written and oral communication skills.Current competency and knowledge of standards of practice, trends, and developments in related scope of job role or practice.Demonstrated excellent customer service through his/her attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization. Ability to fully utilize KJMC's electronic record systems, equipment and other healthcare and billing systems relevant to this position.Spanish speaking preferred.Licenses, Certifications:Current NYS Nurse Practitioner License required.Current NYS RN License required.Current Nurse Practitioner certification in specialty area.Board Certified as a Diabetes Care and Education Specialist (CDCES), preferred.Physical Requirements:Position requires prolonged periods of standing, reaching, walking throughout the working day.Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.Able to lift 100 pounds, if necessary, to assist lifting and moving patients.An individual in this position may be exposed to: adverse indoor and outdoor environmental conditions, Blood borne pathogens, Physical hazards (e.g., needle pricks, etc.), and Chemical hazards.
