Description
Clinician, IAPP - Positive Youth Progression
Licensed Social Workers or Mental Health professionals are eligible for $7500 signing bonus!
The Clinician position will maintain a caseload of eight to ten youth and meet with each on a weekly basis, as part of the Positive Youth Progression program (PYP). PYP provides six to nine months of home and community-based prevention services to youth who are at risk of being removed from their homes. The goals of the program are to prevent out-of-home placement for troubled youth and avoid the juvenile justice system. T his position is working in Catskill, NY and covering Greene County.
Responsibilities:
Identify and assess the strengths, risk factors, and service needs that may impact the well-being and safety of the youth/family, as well as develop treatment goals and methods.
Provide home and community-based clinical treatment coupled with supportive and skill based wrap around casework interventions based on assessment, planning and treatment with youth and families
Provide therapeutic counseling to youth and families which includes a trauma-informed approach to service delivery and working with the clinical case manager to conduct family sessions
Communicate with the youth’s family to monitor progress and support the family’s effort to keep the youth safe
Ensure provisions of regular treatment plan meetings and services plan reviews to assess on going progress and identify any needed modification to treatment planning
Ensure the coordination of services with service providers and collaterals involved with the youth and family
Complete necessary all records and client information in accordance with agency, county, and state policy and funding requirements
Your Experience and Qualification:
Masters degree in social work, mental health counseling, or equivalent from an accredited graduate school program
0-2 years post-masters experience
Experience providing services to teens ages 10-18 with emotional or behavioral needs preferred
Home and community based experience a plus
