This role supports PCPs and Care Teams in coordinating and managing patients in between episodes of care.AREAS OF RESPONSIBILITIESMedical Primary CareSUMMARY OF DUTIES AND RESPONSIBILITIESPhysician Assistants hired for the role of "Care Team Quarterback" will hold a hybrid (partly virtual, partly in person) position to support PCPs and Care Teams in managing the following tasks they relate to team-based care within adult primary care clinic:- Prior Authorizations- Medication refills- Consult recommendation ordering and follow up- Forms follow up and completion- InBasket triage- Abnormal results follow up and management- Infusion center related consents and orders- PCP Vacation InBasket coverage- Patient message responseMinimum Qualifications1. Current New York State registration as a Physician Assistant; and, 2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and, 3.
