Overview

The Physical Therapist evaluates participants aged 55 or older and assesses their mobility level and other functional abilities. This position is responsible for collaborating with the therapy team and other interdisciplinary team members. This will include providing and recommending therapeutic exercise, gait training, the application of modalities and the planning and development of group exercise programs for maintenance therapy of LifeCircles participants.

What you'll doConducts initial and periodic assessments, identifies, and develops a specific plan of care for the rehabilitative needs of the LifeCircles participant and assures all are completed as described in LifeCircles policy.Assesses the need for adaptive and/or durable medical equipment, orthotics and prosthesis, followed by recommendations to the Interdisciplinary Team (IDT).Assesses the need for physical therapy services provided at the LifeCircles Center, followed by recommendations to the IDT.Plans, develops, and implements a plan of therapy to include exercise, treatments, use of modalities, implementation of assistive devices and/or maintenance for participants to meet goals when indicated.Assesses all newly issued orthotic shoes, braces, and prosthetics for appropriateness and proper fit. assists in monitoring diabetic footwear needs.Provides treatment in appropriate settings to meet participant care needs, settings may include participant's home, skilled nursing/assisted living facilities, or LifeCircles center.Participates in the discharge planning of participants from the hospital, assisted living, and/or skilled nursing facilities.Assists with DME management, ordering, installation, and adjustment.Contributes to shared therapy team tasks following department policies and procedures.Communicates with care partners to coordinate care and provide training and education as indicated.Completes documentation per organizational standards.What it takes

Current, unencumbered, active license to practice as a Physical Therapist (PT) in the State of Michigan.Must be comfortable in a self-directed environment.Proficient in computer use: Excel, Microsoft Office, Outlook, and electronic medical records.Why Brio Living Services
