Under the general direction of the Chief Physician & Surgeon, Complex Care, the Advanced / Complex Care Physician & Surgeon (P&S) is assigned to support care teams in the evaluation and management of patients with complex, acute and chronic conditions through provision of clinical consultation and/or facilitation of multidisciplinary forums to assist in developing advance care planning. The P&S serves as a clinical subject matter expert (SME) who develops and updates evidence based clinical guidelines and tools, continuing medical education activities and clinical policies and procedures related to providing care to patients with complex needs. The P&S develops programs designed to identify, treat and monitor complex patient populations. The P&S will support quality improvement activities statewide including developing clinical toolkits and change packages and educating and supporting institution-based providers in providing value-based patient care using the tools created. The P&S serves as a clinical consultant/SME for clinical projects and performs clinical practice reviews and mortality reviews as needed. Moderate travel is required.

Experience in Quality Improvement and Disease Management programs. Experience in Program Development/Population Management in a Public Health setting and/or Masters of Public Health degree. Strong clinical care background/clinical skills preferred. Additional subspecialty board certification and/or extensive clinical experience in Pain Management, Palliative Care, Addiction Medicine, Geriatrics, or Hospital Medicine is a plus, but not required.
