Primary Care PACE Physician Join our robust team as a primary care provider for a San Diego based Program of All-Inclusive Care for the Elderly (PACE), providing high quality, compassionate geriatric care in an outpatient setting. Opportunity for Loan Repayment or Loan Forgiveness Job Description -Provides acute, chronic, preventive, curative and rehabilitative medical care to patients -Manages transitions of care, chronic disease, advanced goals of care discussion and palliative management -Refer participants to other in-house providers, including behavioral health -Document visits in the electronic medical record Hours -Monday through Friday -Full time Preferred - 1 year post completion of clinical training - Employment experience in a community health center - Experience working with elderly population - Medical Spanish preferred - Leadership experience a plus!
