Benefits Eligible Full-Time Benefit Eligible Work Shift Day - 8 Hour or less Shift (United States of America) Department 8750 Quality & Patient Safety The Clinical Practice Guidelines (CPG) Program Manager oversees the coordination and implementation of evidence-based clinical practice guidelines within Kaweah Health, focused on the CMS populations of Pneumonia, Heart Failure, COPD and Sepsis. This role involves working closely with clinical teams to ensure the integration of guidelines into daily practice, conducting audits for adherence, and facilitating education sessions for staff. Also plays a key role in evaluating the effectiveness of guidelines through outcomes assessment and quality improvement initiatives. QUALIFICATIONS License /Certification Required: CA Registered Nurse License Education Preferred: BSN, MSN Experience Preferred: Five years of acute care nursing experience Knowledge/Skills/Abilities Highly developed group facilitation skills to build consensus JOB RESPONSIBILITIES Essential Coordinates the implementation and revision of clinical practice guidelines across different patient populations targeted by CMS including COPD, Pneumonia, Heart Failure and Sepsis. Conducts systematic literature searches, and creation of evidence summaries for working groups and assist them to develop guidelines, workflow/algorithms, supplementary tools. Guides and facilitates interdisciplinary workgroups to appropriate and useful outcomes. Actively engages and creates collaborative relationships with physicians and other health care professionals within interdisciplinary work groups in the inpatient, ED and ambulatory settings to develop guidelines. Ensures active input and agreement of stakeholders to support utilization of guidelines. Responsible for project management (planning and execution) of clinical practice guidelines within a specified time frame as assigned by the Director of Quality & Patient Safety: Guides and facilitates interdisciplinary workgroups to appropriate and useful outcomes. Actively engages and creates collaborative relationships with physicians and other health care professionals within interdisciplinary work groups in the inpatient, ED and ambulatory settings to develop guidelines. Responsible for developing agendas and overseeing the completion of deliverables for all regularly scheduled meetings (work groups, leadership teams, etc.). Creates and sustains a participatory environment. Monitors and coordinates progress of work groups to ensure on time completion of work product. Uses advanced facilitation skills, as demonstrated by successful completion of complex tasks involving workgroups. Includes ability to prevent, diffuse or resolve conflict in a positive manner. Maintains constant communication and updates on guideline status and any issues related to any of the initiatives with all stakeholders and leadership. Conducts audits and assessments to monitor compliance with guidelines and identifies areas for improvement. Assists in monitoring and evaluating patient care in relation to guideline development, implementation and maintenance. Partners with Kaweah Health ISS informatics department and other Quality staff to design and implement the performance measures and productivity data collection, analyses and reporting processes for guideline utilization. Develops and delivers educational programs on new guidelines and updates to healthcare providers. Provides consultative services to groups regarding grading of evidence and the creation and maintenance of guidelines at Kaweah Health. Actively participates in the development and implementation of the Kaweah Health Clinical Practice Guideline Program strategic plans, operational goals, and objectives, business initiatives and policies and procedures. Additional Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
