OBJECTIVE/PURPOSE:
Located at SASH affordable housing site (s) and an active member of the SASH team including the SASH Coordinator and Wellness Nurse (RN), the Clinician provides whole-person care coordination and support targeted to individual participants as well as the SASH population as a whole.
MAJOR RESPONSIBILITIES:
1.0 DIRECT SERVICE:
Direct individual therapeutic support provided in-person, by phone and through telehealth including individual, couples and family therapy, and other psychosocial support.Assessment, screening, and triage services to help identify unaddressed needs and to assist individuals in connecting to clinician and/or other community resources and providers.Group-based programs on topics relating to mental health, substance use, aging, and other emotional needs.Additional Direct Services may include Conflict mediation sessions; Support with completing Advance Directives and obtaining DNR/COLSTs; Support with housing retention.
2.0 CARE COORINATION:
Care coordination to include regular consulting meetings with embedded SASH staff, monthly SASH team meetings, and outreach to primary care, psychiatric, and other community partners and service providers.
3.0 CONSULTATION AND TRAINING:
Provide training and consultation to SASH Coordinators and Wellness Nurses addressing challenges confronted by SASH participants with mental illness, substance misuse and other social, behavioral, or emotional concerns.Develop and present trainings and workshops to UCS or SASH staff or to other populations as identified by the team.Other needs as identified by staff, residents or through direct observation of need.
4.0 ADMINISTRATIVE:
Utilize SASH data management system (s) in order to collect and present outcome data for the program.Accurately completes timesheets and expense reimbursements on a timely basis.Performs other administrative tasks as required.
Qualifications
EDUCATIONAL & EXPERIENCEREQUIREMENTS:
Masters Degreein counseling, social work, psychology, or closely related field.Licensed OR on the roster of Non-licensed Non-certified Psychotherapists in Vermont.Valid Drivers license and use of a personal vehicle.
