9 Panel completed within 30 days of start date, preferred within 14 days of start.Must be completed and resulted before start with MRO signature.Must include the following: Cocaine, Amphetamines, Opiates, 6- Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, and Methadone. (NO THC).All engaged contractors must have signed forms on file in Trinity VMS, including Trinity Code of Conduct Form, Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement, Trinity Integrity and Audit Services Form, Trinity I9, Trinity MVR (Motor Vehicle Report).Proof of vaccine, declination or exemption(with agency letterhead).Agency will provide Trinity Skills Checklist for clinical contractors.
