Job Description:
- Conduct thorough assessments of patients' physical conditions and functional limitations.- Develop individualized treatment plans based on patient assessments and goals.- Implement physical therapy interventions, including exercises, manual therapy, and modalities, to improve patients' mobility, strength, and functional abilities.- Educate patients and their families on treatment plans, home exercises, and ways to improve overall wellness.- Monitor and document patients' progress, adjusting treatment plans as needed to ensure optimal outcomes.- Collaborate with interdisciplinary team members, including physicians, occupational therapists, and nursing staff, to ensure coordinated and comprehensive care.- Participate in team meetings, case conferences, and continuing education opportunities to stay current with best practices and advancements in physical therapy.- Maintain accurate and timely documentation of patient care in compliance with regulatory and facility standards.- Adhere to ethical and professional standards set by the American Physical Therapy Association (APTA) and state licensing board.
Physical Requirements:
- Ability to lift and move patients, equipment, and supplies.- Prolonged periods of standing, walking, and performing physical therapy interventions.- Manual dexterity and strength to perform therapeutic exercises and manual therapy techniques.
