This is a great opportunity for an experienced Registered Nurse to work close to home at a 3-time Magnet -recognized community hospital.
The Acute Care RN Navigator under the supervision of the Director of Continuity of Care, performs/supervises services according to the policies, procedures, philosophy, and objectives of the department and the hospital.
The Navigator works cooperatively within the hospital inpatient departments and other services to create a system of quality healthcare. Serves as a liaison between the patients to community resources, providers, and supports a smooth transition from hospital to home with a goal of eliminating and/or reducing readmission, patient satisfaction and continuity of care across the continuum.
The Acute Care Navigator has critical thinking skills and has the ability to plan, implement and evaluate individual patient care programs, The Navigator has strong communication skills and has the ability to provide emotional stability to cope with human suffering, emergencies and other stressors.Assesses and facilitates coordination of multidisciplinary care across the care continuum.
Provides communication, advocacy, and education in a culturally competent manner.
Triages new patient referrals to ensue timely access to care.
Conducts post discharge follow up phone calls and serves as a liaison and patient advocate in navigating the healthcare system.
Provides patient education and supports utilizing knowledge of adult learning competencies and education resources to support complete, connected, and comprehensive care
Collects and reports data from patient on barriers, hospital experience, health care, medications, prior treatments, etc.
Collaborates with multidisciplinary care team and expedites sequence of patient's care.
Serves as a liaison between patient and providers/clinicians to address patient’s needs
Assesses patient's needs and ensures such needs are integrated into patient’s care plan.
Encourages and empowers patients to actively participate in decision making about their treatment and care.
Hours :40
hours per week: Mon - Fri daysNo nights or weekends are required!
Requirements:Massachusetts RN Licensure.
3 Years of RN Experience.
Current Basic Life Support (BLS) Certification.
Preferred Qualifications:
Bachelor of Science in Nursing (BSN).
