Job Description Summary
Physical Therapists are responsible for providing physical therapy services to patients in their own homes. The PT will assess patients' conditions, develop personalized treatment plans, and deliver therapy to help improve mobility, reduce pain, and restore function. The role involves traveling to patients' homes and adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.
Essential Job Functions/Responsibilities
1.
Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
2.
Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
3.
Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
4.
Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests. Records and reports to the physician any adverse reaction to treatment. Obtains additional orders, if necessary.
5.
Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
6.
Teaches, supervises, and counsels the family and client in the physical therapy program. Instructs client in care and use of adaptive devices such as:
wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
7.
Records on a timely basis all evaluation data, treatments, and client's response to therapy intervention. Also records quality assurance data as directed.
8.
Prepares an evaluation of the client's condition and performs periodic reassessments.
9.
Supervises Physical Therapist Assistants at least every 30 days.
10.
Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary. Participates with staff, clients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary
11.
Supervises the home health aide every 14 days when physical therapy is the only professional discipline involved in the client's care.
12.
Evaluates the client, the client’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other Agency and community services will be required
Awareness of the Agency’s QAPI program. Active participation as required per policies and procedures related to the Agency’s QAPI program.
Acute awareness of patient rights and the Agency’s responsibility in honoring patient’s rights. Awareness of patient rights description in Agency’s service agreement and Agency’s admission packet.
Awareness of appropriate reasons for a patient discharge and Agency’s policy on appropriate reasons for discharge.
Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
Awareness and ability to design and identify patient centered and individualized goals with realistic measurable timeframes
Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency’s policy and procedures. Awareness and participation in the Agency’s infection control program.
Appropriate bag technique policy acknowledgement and demonstrated competence when applicable. Physical therapy equipment and other medical equipment will be utilized. Hand washing materials and PPE.

Position Qualifications

The ability to drive and the ability to access clients’ homes, which may not be routinely wheelchair accessible, are required.
·
Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
·
On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
·
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
·
Must be licensed or registered by the State.
·
Must have at least one year of institutional experience.
·
The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience. (2 years preferred)
Must have one year of experience in an acute/sub-acute setting.
