JOB DESCRIPTION SUMMARY

As a Baylor LPN for Pinnacle Home Care, you will play a crucial role in providing compassionate and high-quality healthcare services to patients within the comfort of their homes. The Baylor LPN will work on a weekend-specific schedule, providing continuity of care and support to patients. This position requires a dedicated and skilled professional who can deliver personalized patient care, administer medications, and collaborate with a multidisciplinary team to ensure the overall well-being of our clients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1.

Carry

out the plan of care under the supervision of the registered nurse.

Assists the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under the physician's orders

Assists with the preparation, implementation, and continuing evaluation of the client care plan

Recognizes and understands the effects of social and economic problems upon clients and provides for the emotional and physical comfort and safety of clients

Helps teach the patient the appropriate self-care techniques

Fosters cooperative effort among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences

Assumes responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning

Prepares clinical and progress notes (not to include admission).

Includes interim orders written with measurable goals and time frames and the integration of these orders into the patient’s plan of care under the supervision of a Registered Nurse.

Observes, records, and reports to the appropriate person patient symptoms, reactions and changes including:

a.

The general physical and mental condition of clients and signs and symptoms which may be indicative of untoward changes

b.

Stresses in human relationships between the clients and staff personnel and between clients and their families and visitors

Observes, records and reports to the appropriate person the general physical and mental condition of clients and signs and symptoms which may be indicative of changes in condition

Assists with the rehabilitation of clients according to the medical care plan by encouraging the interests and special aptitudes of the client and encouraging clients to help themselves using their own capabilities

Further assists in rehabilitation by reporting any abnormality in the client’s range of motion, body mechanics or body alignment.

Actively participates in nursing organizations and promotes and participates in in-service programs.

Awareness of the Agency’s QAPI program.

Active participation as required per policies and procedures related to the Agency’s QAPI program.

Management and follow-up of customer complaints, following the Agency’s complaint policy.

Acute awareness of patient rights and the Agency’s responsibility in honoring patient’s rights.

Awareness of patient rights description in the Agency’s service agreement and Agency’s admission packet.

Awareness of appropriate reasons for a patient discharge and the Agency’s policy on appropriate reasons for discharge.

Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.

Awareness and ability to design and identify patient-centered and individualized goals with realistic measurable timeframes under the supervision of a Registered Nurse.

Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency’s policy and procedures.

Awareness and participation in the Agency’s infection control program.

Nursing bag technique policy acknowledgment and demonstrated competence.

Utilization of CRT, calculator, multi-line telephone, copy machine, and basic medical equipment.

The ability to communicate well, both verbally and in writing is required.

The ability to access clients’ homes which may not be routinely wheelchair accessible is required.

TREATMENTS AND PROCEDURES

The Licensed Practical/Vocational Nurse with the education and proven clinical competence may perform certain treatments/procedures provided the Home Health Agency’s policies and the State’s Regulatory Acts allow their performance. Only advanced LPNs with IV certifications can perform IV-related procedures.
