Job Description:
- Provide care to assigned residents in a caring, safe, and efficient manner according to standards, procedures, and individualized care plans. - Complete comprehensive nursing assessments of all residents on admission and ongoing as per state and federal regulations using the nursing process. - Demonstrate competence in clinical skills as identified on Competency Verification and Training Checklist. - Ensure assigned residents receive ordered medication, treatments, and procedures within defined time frames using appropriate infection control procedures. - Demonstrate familiarity with the therapeutic effects and side effects of all medications administered. - Obtain equipment and supplies as needed to provide care. - Follow procedures for managing pressure ulcers, dehydration, malnutrition, incontinence, pain, and medication administration. - Accurately complete glucometer testing and control monitoring as per procedure. - Implement specific approaches to manage resident behavior and enhance mood state per resident care plans. - Review and update resident care documentation as necessary using the EMR accurately. - Document actions and observations related to resident condition changes per policy. - Record daily skilled nursing notes and other actions related to admission, discharge, transfers, or death. - Document incidents or accidents in resident records and complete necessary forms. - Complete behavior documentation and seven-day observation tools as required. - Participate in the quality improvement process, including audits and corrective procedures as assigned. - Complete portions of the Minimum Data Set (MDS) for Nursing and sign the forms as necessary. - Ensure physician orders are processed, and notify physicians of necessary updates in a timely manner. - Conduct rounds with physicians when assigned. - Attend or obtain resident reports before the start of shifts, exchange relevant information, and observe and report resident conditions. - Respond promptly to resident call lights and assist with resident and family education. - Notify family members of condition changes and assure timely follow-up of any signs or symptoms of resident infections. - Comply with all related laws, regulations, company policies, and procedures, fulfilling obligations under the corporate compliance program. - Supervise assigned employees, including interviewing, hiring, training, planning, assigning, and directing work, appraising performance, rewarding and disciplining employees, addressing complaints, and resolving problems. - Maintain a clean and safe working environment with a moderate noise level. - Adhere to weight-lifting requirements, needing to lift up to 35 lbs. - Cover every third weekend and may pick up additional shifts if available. - Rotate holiday coverage, with availability to pick up additional shifts.
